In 2008, Adam sent Josh an image of an anime style girl sitting down and holding a plush doll. Both agreed the smiling girl was adorable. Next to the girl was yellow text that read "Maybe perfect sister!?". Adam and Josh found the Engrish amusing and pondered the notion that maybe she was, indeed, a perfect sister.

Since then, whenever Adam and Josh encounter a sister in an anime who appears particularly considerate, reliable, or responsible, one of them poses the question "Maybe perfect sister!?". The first character to be put under the Maybe Perfect Sister!? microscope was Ui from the series K-ON! Another notable Maybe Perfect Sister!? candidate is Nori Sakurada from the series Rozen Maiden.

Sometime in 2011, Adam and Josh were driving around when the Stevie Ray Vaughan song "Look at Little Sister" came on the radio. Adam sang along with the chorus, changing the lyrics to "Hey hey heeeey hey, ooo, maybe perfect sister!?". Henceforth, any time the song is played, Adam and/or Josh sing the amended version. They also often reference the song when a normal Maybe Perfect Sister!? situation presents itself.
